This entry form constitutes a formal agreement between the Team Captain and Twilight League. The agreement lasts for a full season, or the remainder of the season if your team join midway through a league season. Any team dropping out of the league or removed for disciplinary reasons will still be liable for the outstanding balance for the remainder of the season.

It is the responsibility of individual team members to arrange their own personal injury insurance. It is recommended that you arrange your own personal injury cover for injuries sustained during play.

I understand that both myself and my team are playing at our own risk in any Twilight League organised events and Twilight League cannot be held responsible for personal injury sustained whilst participating in the leagues.
